Functional Home Evaluation
A comprehensive assessment of current health, treatment, functioning and safety in the home environment. It typically takes three hours to complete, and can be broken into two sessions, or one in-person and one virtual session if indicated. A thorough report is written, including recommendations to maximize functioning.
Health Status
Physical, psychological, cognitive and social issues. Hands-on assessment, self-report measures and standardized tests. Past and current treatment.
Functional Status
Participation in self-care, productive and leisure activities. Identification of past performance and future goals. Use of self-management strategies to maximize functioning. Observation of functional tasks, self-report and standardized tests.
Recommendations
Clinical findings, test results, nature of functional challenges, reliability of effort, possibility for functional improvement, ability to return to work, relevant treatment recommendations and work accommodations.
